Here are some of the affiliate programs that I have used to make money blogging. I will be adding to and updating this list with my experience with these and any new programs out there for making money online so check back often!
Blogging to make money:

Write blog posts about selected advertisers. Pays per post. Minimum payout $50.
Automatically affiliates outbound links in your site so you get paid when your readers click through and make a purchase. No minimum payout. Paid monthly.
Make money with advertisements:
Ad network for search targeted, mobile and local ads that are suited to your audience’s interests. Minimum payout $10 via PayPal.
Make money with email, textual, banner & other links placed on your website. Pay per order or pay per lead programs available. Minimum payout $1.
Find a sponsor for your web site. Get paid for your great content. shareasale.com. Minimum payout $25.
Make money with in text ads:
Automated in-text pay-per-click advertising. Minimum payout $50
Make money on Twitter:
Share with your audience on Twitter, Tumblr and/or YouTube. Minimum payout $1
Post sponsored tweets on Twitter. Minimum payout $50, or under $50 with $2 fee.
Make money by surfing the internet or other things:
Earn SwagBucks to redeem for prizes. Earn bucks by doing web searches, completing offers or other things.
Earn Hulk Coins to redeem for Amazon codes or other prizes. Get 50 Hulk Coins for signing up.
Make money by when you’re shopping online:
Visit EBATES before shopping online to earn money back on your purchases. Once you earn more than $5.01, your payment will be sent in the next quarterly payout. I’ve used them for years. Love EBATES!
Click through BeeSavy when you’re shopping online to earn up to 35% cash back.

Its a great list you have prepared! I also have a blog and that is one thing i haven’t done is a list like yours to help people out. I wonder if you may know if there is an affiliate company for Facebook, especially since a lot of the free samples and giveaways are mostly there now.
